Dental Insurance & Billing Policy

At Hoke Family Dentistry, our goal is to help you maximize your dental insurance benefits and make receiving quality dental care as simple and affordable as possible. Dental insurance is a contract between you, your employer, and the insurance company, but we are here to help you navigate your coverage.

1. Insurance Plans We Accept

We bill all insurance plans as a courtesy to our patients. Some of the common insurance providers we process claims for include:

  • Delta Dental
  • Cigna Dental
  • MetLife Dental
  • Aetna Dental
  • Blue Cross Blue Shield (BCBS)
  • UnitedHealthcare (UHC)
  • Guardian Dental
  • Humana

Note: Since plans vary significantly based on your employer’s agreement, we recommend calling our office prior to your appointment to verify if your specific plan is accepted.

2. In-Network vs. Out-of-Network

We are out-of-network, but YOU CAN STILL RECEIVE CARE AT OUR CLINIC! As a courtesy to all our patients, our administrative team will file all claims and submit documentation directly to your insurance company on your behalf to ensure you receive the benefits you are entitled to.

3. Co-Payments, Deductibles, and Patient Responsibility

Please keep the following financial policies in mind:

  • Estimated Patient Portion: You are responsible for any co-payments, deductibles, or non-covered service percentages at the time of your treatment.
  • Estimates Only: Any estimate we provide regarding your insurance coverage is an estimate based on information provided to us by your insurer. It is not a guarantee of payment.
  • Final Balance: You are ultimately responsible for the full cost of services rendered in the event that your insurance denies coverage or pays less than estimated.

4. Payment Methods & Financing Options

For your convenience, we accept cash, personal checks, major credit cards (Visa, MasterCard, Discover, Amex), and debit cards. If you need assistance paying for treatments not covered by insurance, we also work with third-party financing solutions like CareCredit to offer flexible, interest-free payment options.
